All prepaids work the same way.
Since I don't know, from which country you are I try to explain it in general:
You get the SIM (with or without new phone - depends on you)... normally they give you starting credit (Germany: 10-15 Euro), which you can use.
Once the credit is low, you get a voice message when make a call or a text message with a warning.
Now you need to recharge your account.
There are many ways. Here in Germany you can charge the account via Visa card, bank transfer or by buying a card with credits.
With these "credit cards" charging is the following:
You've got to call you account manager (which is for free), browse throught the menu and have to enter a sequence of numbers. When the number is confirmed (mostly within seconds) your new credit will be credited.
(Some providers also alow you to enter these sequence of numbers in to phone with some other sequence additions and "send" it somehow - I don't know how to descripe it in english nor the english word for it).
By calling this account manager you also can check your current credit.
(can also be done by an easy key sequence)
When you get a prepaid package, you also get many informative booklets which explain everything which you need to know for the future.
Prepaid is recommendable for kids and people who primary write alot text messages.
Outgoing calls are quite expensive.

Do I have to pay extra to make an Account, which I propably would have to do? Or is that already included in the price?

That's already included in the price. You get an account, phone, phone number, SIM, and some starter minutes. I work at a tmobile, and I know the prepaid phones we give out come with 150 mins that expire after 60 days.
