Dear Sir,

A really true step by step guidance. Just to be double sure, in step 2/3,
2. Take out old MMC
3. Put new MMC to the phone and format the new MMC (with the phone) and rename it the same

This mean after taking out the old MMC, you put a new MMC to the phone and switch it on. Doesn't that give any conflict since most of the software have been installed in the old MMC card and you put in an un-formatted MMC card to the phone.

Can you format the new MMC card via the MMC reader or is it not advisible to do so?

When formatting with the phone all necessary folders will be created automatically. And there won't be any problem with the files system (the phone only can FAT(16) but Windows sets as standard FAT32 which isn't useable by the phone and that's why many people have problems).
